Primary Care Physician office visit co-pay increases from $15 to $20
Specialist office co-pay will be $25
Deductible for services other than office visit
In-network: $300 Individual /$600 Family
Out-of-network: $600 Individual / $1,200 Family
Emergency Room Services: $35 co-pay (co-pay waived if admitted)
Retail Co-pays (up to 30-day supply): $7 Generic / $15 Preferred Brand / $20 Non- Preferred Brand
Mail Order or CVS Pharmacy Co-pay (up to 90-day supply):
$10 Generic / $25 Preferred Brand / $35 Non-Preferred Brand
Mandatory Generic for Mail Order or CVS Pharmacy 90-day supply: If you or your doctor requests the Brand name when a Generic is available, you will pay the Brand co-pay plus the difference in cost between the Brand name Generic.
The maximum coverage per person for dental will increase from $2,000 to $2,250 per calendar year.
The maximum coverage for orthodontia for children under the age of 19 will increase from $2,000 to $2,250 per life
The out-of-network frame allowance will increase from $60 to $75 every 2 years.
This increase will also apply to in-network claims, covering 85% of manufactured frames in full (additional charges for special lenses, coatings, etc. will continue to apply)
BASIC LIFE INSURANCE
Increases from $40,000 to $50,000
ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE
Increases from $7,500 to $50,000
OPTIONAL LIFE INSURANCE
Employees may purchase at their cost, additional Optional Life Insurance in amounts of $50,000, $100,000, $150,000, or $200,000. Premiums are based on tobacco/non-tobacco rates within age brackets, and payroll deducted.
